As Personal Support Manager, you will coordinate and supervise Community Health Services delivery. You will coach Community Support Workers in the field and develop effective Care Plans independently or with Ontario Health atHome. The role also includes performance monitoring, workplace health and safety guidance, and public relations.
Key Responsibilities: Coordinate Community Health Services delivery across assigned areas
Supervise and coach Community Support Workers in the field
Develop and maintain effective Care Plans
Monitor employee performance and applicable policies
Guide workplace health and safety activities
Requirements: Registered Nurse or Registered Practical Nurse degree or diploma
Registration with the College of Nurses of Ontario
Three to five years of job-related experience
Knowledge of case management and nursing practices
Current CPR Certification, vehicle, and regular travel availability
Bring nursing knowledge, leadership, and care coordination skills to support CarePartners patients and Community Support Workers.
