WellFort seeks full-time and part time Primary Care Physicians for Bramalea, Four Corners, and future Peel Region sites. Physicians assess patients, treat acute and chronic conditions, and collaborate within interdisciplinary teams. The work includes outreach, advocacy, education, and quality improvement.
Key Responsibilities: Provide accessible primary care in clinics and by telephone
Assess patients and diagnose acute and chronic conditions
Coordinate care through case conferences and consultations
Participate in outreach, education, and prevention programs




Provide rotating on-call coverage throughout the year
Requirements: Good standing with the College of Physicians and Surgeons of Ontario
Membership in the College of Family Physicians of Canada
Eligibility for required CMPA malpractice coverage
Interdisciplinary practice experience and strong communication skills
Electronic Medical Record proficiency and confidential information experience
Support community-oriented primary care at WellFort through clinical assessment, collaboration, prevention, and continuity across Peel Region.
