This Nurse Practitioner supports an innovative residential mental health program for young adults with serious mental illness. The position delivers recovery-oriented care within a team-oriented, multidisciplinary team. Core work includes health assessments, individualized care planning, crisis intervention, and preventative health support.
Key Responsibilities: Complete comprehensive health and mental health assessments
Diagnose and manage acute and chronic health conditions
Prescribe medications and monitor treatment plans
Develop individualized care plans for clients




Provide crisis intervention and risk assessments
Requirements: Current CNO registration in good standing
Mental health, addictions, community, or primary care experience
At least four years of clinical experience outside school
Strong assessment, diagnostic, and medication management skills
Experience collaborating within interdisciplinary healthcare teams
