You will assess patients, coordinate interventions, educate families, and collaborate with an interprofessional care team.

Key Responsibilities: Assess outpatient patients using observation, communication, data review, and physical assessment Plan, provide, evaluate, and document nursing care Teach illness management, prevention, treatment, and self-management skills Coordinate care plans, referrals, discharge planning,
