As a Care Coordinator (Status: Full-Time, Start date: Immediately; Location: Parry Sound -West Parry Sound Health Centre) , you will assess and determine patient careneeds and eligibility, provide access and referrals to community services, andengage with patients, caregivers and other health care practitioners. In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans Coordinate and monitor care plan delivery Establish a helping relationship with patients and their families Initial assignment Hospital, withpossibleassignmentchanges within the occupation based on operational needs Membership, in positive standing, with the applicable regulatory body: College of Nurses of Ontario Ontario College of SocialWorkers and Social Service Workers Recentexperience in community health or a related field Knowledgeof the health care delivery system and community resources Advance oral and writtenproficiency in English is essential We have amandatory COVID-19 vaccination policy.
