Support substance use recovery through case management and harm reduction with House of Friendship across Waterloo Region. This is a full-time, salaried position. As a Connection Coordinator, you will work in the HART Hub program supporting people facing homelessness, mental health concerns, and addiction.

You will use person-centred care, Motivational Interviewing, crisis intervention, and community referrals. The work includes independent community travel and collaboration with LANGS HART Hub and Interprofessional Primary Care teams in Cambridge.
