Fraser Health seeks a Home Health Clinical Social Worker skilled in psychosocial assessments, counselling, and interdisciplinary care planning.

Reporting to the Team Leader, this Social Worker provides therapeutic clinical social work services to Home Health clients and caregivers. The position addresses significant psychosocial problems through assessments, interventions, family counselling, consultation, and referrals. It also supports education, research, and quality improvement with interdisciplinary teams.

Key Responsibilities:

- Conduct psychosocial assessments and recommend treatment plans
- Provide therapeutic interventions, family counselling, and follow-up
- Consult with clients, caregivers, teams, and community agencies
- Facilitate referrals, education sessions,



and community linkages
- Maintain records, evaluate care, and participate in research

Requirements:

- Bachelor's Degree of Social Work from an accredited school
- Three years' recent community client and caregiver experience
- Current full registration with the BC College of Social Workers
- Valid BC Driver's license and personal vehicle access
- Ability to communicate effectively verbally and in writing
