Job Description

We are seeking RPN's to join our community based team! The RPN will Report to the Registered Nurse and will support individuals within their homes requiring dialysis.

Duties and Responsibilities:

- Complete 4-5 week home hemodialysis training program
- Evaluate patients before, during, and after dialysis treatment (vital signs, weight, fluid status).
- Monitor for any changes in condition or complications during treatment
- Set up and operate dialysis machines.
- Insert and monitor vascular access (fistulas, grafts, or central venous catheters).
- Start and terminate dialysis treatments safely and efficiently.
- Continuously observe patients for signs of distress or adverse reactions.
- Monitor fluid removal, machine pressures, and blood flow rates.
- Administer medications such as anticoagulants (e.g., heparin) as ordered.
- Manage and document medications given during dialysis.
- Accurately record treatment data, patient responses, and any incidents.
- Maintain detailed nursing notes for interdisciplinary care planning.
- Inspect, calibrate, and clean dialysis equipment regularly.
- Follow infection control and safety protocols strictly.
- Teach patients about dialysis, kidney health, fluid and dietary restrictions, and medication adherence.
- Support patients emotionally and psychologically, helping them adjust to long-term treatment.
- Coordinate with other healthcare providers for patient follow-up and home care support when necessary.
- Collaborate with interdisciplinary team such as nephrologists, dietitians, social workers, and other nursing staff.
- Adhere to regulatory standards (e.g., infection control, patient privacy).

Qualifications:

- Registration with the College of Nurses of Ontario as a Registered Practical Nurse (in good standing);
- Current B.C.L.S;

· Minimum one-year experience

- Related experience in a geriatric or Long-Term Care environment is preferred
- BScN preferred;
- Demonstrated commitment to maintain Reflective Practice Standards as outlined by the CNO;
- Demonstrated knowledge of and compliance with safe work practices;
- Must possess strong written and oral communication, critical thinking, care coordination, time management and customer focus skills;
- Demonstrated empathy, concern and interest in providing quality care and service for residents;
- Physical ability to perform the essential duties of the job;
- Must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months;
- Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy;
- Demonstrated understanding of and commitment to The Lighthouse Retreat INCs. comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area; and;
- Proven valuable attendance record with capability of maintaining this same standard;

Salary:

RNs/RPNs: $30 - $35 per hour
