Job Summary

Exacta is seeking a compassionate and reliable Personal Support Worker (PSW) to provide short-term personal support to a male claimant in Chatham, Ontario.

The client is a wheelchair user and requires assistance with personal care and mobility. He wears incontinence products, has limited use of his hands, has memory/cognitive difficulties, and requires assistance due to his larger body size.

The assignment includes approximately 2 hours of support at a medical appointment in Windsor, Ontario, as well as travel time to and from the client's long-term care residence in Chatham. Transportation arrangements and pickup time will be provided by Exacta.

Responsibilities

- Provide respectful, compassionate, and professional personal support to the client.
- Assist the client with safe transfers, mobility, and positioning as required.
- Provide assistance with toileting and incontinence care as appropriate.
- Assist the client with getting ready for and attending the medical appointment.
- Accompany the client during transportation between the long-term care residence and appointment location.
- Provide appropriate physical assistance while respecting the client's dignity, privacy, and independence.
- Monitor and respond appropriately to the client's memory/cognitive difficulties.
- Follow all care instructions and safety protocols provided by Exacta and the applicable care team.
- Communicate any concerns, changes in the client's condition, or incidents to the appropriate Exacta contact.
- Maintain confidentiality and treat the client with dignity and respect at all times.

Requirements

- Valid Personal Support Worker (PSW)



certification or equivalent relevant experience, as required for the assignment.
- Previous experience providing personal care to clients with mobility limitations and complex support needs.
- Experience assisting wheelchair users and with safe transfers is strongly preferred.
- Comfortable providing incontinence and personal hygiene assistance.
- Able to safely provide physical assistance to a larger-bodied client and follow proper body-mechanics and transfer procedures.
- Experience supporting individuals with memory or cognitive difficulties is an asset.
- Compassionate, patient, dependable, and skilled manner.
- Strong communication and interpersonal skills.
- Ability to maintain client confidentiality and professional boundaries.
- Reliable transportation and punctuality are essential for this assignment.
- Ability to accompany the client during transportation from Chatham to Windsor and return.
- Must be physically capable of performing the essential duties of the assignment safely.

Preferred: Experience working in long-term care, home care, rehabilitation, or with clients requiring wheelchair, transfer, and personal-care assistance.
