Deliver patient-centered pharmacy care across Kitchener, Waterloo, and the GTA using clinical expertise, counselling, and medication management skills. As a Regional Relief Pharmacist, you will support assigned pharmacies across the territory. You will provide professional clinical services, patient education, and customer service. The work includes disease state management, medication reviews, adherence support, health risk assessments, and injections.

Key Responsibilities

- Provide compassionate patient-centered care within the assigned territory
- Counsel patients and caregivers about new therapies
- Initiate care plans, follow up, and document properly
- Deliver medication reviews, injections, and health risk assessments
- Maintain prescription safety and regulatory compliance Requirements:
- Bachelor of Science in Pharmacy from a recognized institution
- Licensed Pharmacist in positive standing within the responsible province
- Injection and First Aid Certification
- Kroll software experience preferred
- Current pharmacy practice and regulatory knowledge Bring clinical pharmacy expertise, patient counselling, and medication management skills to regional relief coverage across the Kitchener-Waterloo to GTA territory.
