We are looking for a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team and provide exceptional nursing care to our clients in a facility setting in Edmonton and surrounding communities.

Position Summary

The Licensed Practical Nurse will provide professional nursing care to clients in their homes while promoting safety, dignity, independence, and quality of life. The successful candidate will work collaboratively with clients, families, caregivers, and the InCasa Home Care team to ensure individualized care needs are met.

Key Responsibilities

- Provide safe, compassionate, and client-centered nursing care in the facility setting
- Complete nursing assessments and monitor clients’ health status
- Administer medications and treatments as directed and within LPN scope of practice
- Monitor and document changes in clients’ condition and report concerns appropriately
- Assist with wound care, catheter care, diabetes management, and other nursing procedures as required
- Support clients with activities of daily living when clinically appropriate
- Develop and follow individualized care plans
- Maintain accurate, timely, and confidential clinical documentation
- Communicate effectively with clients, families, physicians, healthcare professionals, and agency management
- Provide education and support to clients, families and facility staff regarding client care needs
- Promote fall prevention, infection prevention, and overall client safety
- Follow all applicable professional standards, legislation, policies,



and InCasa Home Care procedures
- Respond appropriately to changes in client condition and escalate concerns when necessary
- Provide guidance and support to Health Care Aides and caregivers when required

Qualifications

- Current Licensed Practical Nurse (LPN) registration in Alberta
- Graduate of an approved Practical Nursing program
- Current CPR/BLS certification
- Valid Criminal Record Check with Vulnerable Sector Check
- Strong clinical assessment and documentation skills
- Excellent communication and interpersonal skills
- Ability to work independently in facility setting
- Reliable, professional, compassionate, and dependable
- Previous home-care or community nursing experience is an asset
- Valid driver’s license and reliable transportation are an asset
