Who We Are Looking For
Do people naturally open up to you? Have you been told that you bring calm to chaos? Are you the type of person who shows up fully present for others, not because it’s your job, but because it’s who you are?
You’re a people-first kind of nurse. You see the whole human, not just their diagnosis, and you’re guided by the belief that dignity, joy and deep human connection should be part of every care plan. Your clinical training gives you confidence, but it’s your kindness, empathy and quiet leadership that make people trust you.
At Hero Home Care, we believe that there is an art and a science to caring, and our core purpose is “Happy Humans and Healthier Communities.” That’s not just a slogan. It’s a call to action, and we hope you are the kind of person who answers that call with both heart and hustle. You are proud to do hard things for the right reasons. You don’t shy away from complicated situations or tough conversations. You show up, you listen and you lead with compassion.
You’re the kind of nurse who sees the details others might miss. You care enough to follow through, make one more phone call or take the time to truly understand a family’s needs. You like structure and systems, but you’re also flexible. You know that great care isn’t always predictable, and you’re ready to adapt.
You also understand that helping a family choose care is an important part of caring. You are comfortable explaining our services, communicating their value, answering questions and following up thoughtfully. You build relationships that help the right families choose Hero Home Care and feel confident staying with us.
Whether you’re building a care plan, supporting a caregiver—we call them Heroes—or helping a family navigate change, you’re steady, warm and intentional. You take ownership of care quality, team performance and the relationships that help your community grow. You believe that how we do things matters. You want to create more Moments That Matter, and you want to make them count.
A Day in the Life
There may be no such thing as a “typical day” in the life of a Care Manager, but if there were, it might begin like this:
You review your emails, check in with the team and look at the day ahead. Your schedule is full, but your purpose is clear. You’re here to create more moments that matter for families facing one of the most important decisions they’ll ever make.
Your first visit is with a family considering care for their mother, who has recently had a fall. They’re worried and overwhelmed, but that’s where you shine. You arrive with clinical expertise and calm. You listen deeply, noticing what’s said and unsaid. You ask thoughtful questions, assess her needs and create space for emotion.
You offer solutions tailored to her health, wellness and personal goals. You explain how Hero Home Care can help, answer the family’s questions and agree on the next steps. After the visit, you document the conversation and follow through so the family feels supported as they make their decision.
Mid-morning, you join the company-wide daily huddle from a safe location. It’s a quick seven-minute touchpoint where the team shares good news, celebrates milestones, recognizes our Heroes and aligns on the day’s priorities.
Throughout the day, you might connect with our Care Hub about coverage, update a care plan, speak with a family about changing needs or visit a Hero in the field to provide training and feedback. You balance clinical oversight with coaching and mentorship, helping our team deliver on the promise of exceptional care.
You may also connect with a discharge planner, visit a community partner or follow up with a prospective client who wasn’t ready to begin care. You understand that trust grows through consistent, thoughtful attention.
Your week combines community visits, office collaboration and home-office administration. You typically spend two days in the office and three days in the community, working across Vancouver, Richmond and Burnaby, with office time in Vancouver and on the North Shore.
Behind you is a team that shares your passion. Your Regional Director provides direction, support and accountability, while Hero Central and the Care Hub help you coordinate care and resolve issues.
This is meaningful, rewarding and deeply human work. And you wouldn’t have it any other way.
Our Core Values
People Come First: We don’t just recruit caregivers, we hire Heroes.
Because we know it’s people that make the home care business tick. We put people first—our team members and our customers. We treat all people equally and celebrate diversity. We communicate openly and honestly. And when it comes to our team members’ or customers’ health and happiness, we keep the focus clear.
Commit To It: We’re in the business of caring for others, so that means we’re in the business of making quality decisions and doing whatever it takes to fulfill our purpose of creating happy humans and healthy communities. We do what’s right, not what’s easy. We also appreciate that at the community level, we’re only as good as our word. We see things through and walk our heroic talk.
Go Above & Beyond: Our Heroes know they will be called upon to deliver above and beyond. But it’s not just about energy—which we have a lot of! It’s about inspiration. We think creatively and bring joy to the work because we love serving others and the community. Some days it’s about preparing a delicious meal; other days it’s about replacing the elastic waistband in a customer’s favourite pajamas. True story.
We Do 1% Better: In life, there are rarely silver bullets and one-size-fits-all solutions. So instead, we commit to the journey of continuous improvement. Lasting change through small improvements, one day at a time. We want to lead our industry because our industry needs a Hero. We are open to coaching and receiving feedback because when we grow, our customers and communities do better.
If this feels like the kind of team you’ve been looking for, we’d love to meet you.
