What You Will Do:

- Deliver specialist diabetes assessment and management to adult patients, focusing on Type 2 diabetes.




- Utilize your skills to support Type 1 diabetes management, particularly in non-complex cases.
- Craft and reassess individualized care plans, supporting positive health outcomes.
- Educate patients on self-management strategies, empowering them to optimize their lifestyle.
- Monitor and adjust care plans to meet evolving clinical needs following current guidelines.
- Collaborate with a multidisciplinary team and medical consultants to maintain high-quality patient care.

- Current NMC registration with robust nursing credentials.
- Significant experience in diabetes care, preferably in a specialist service setting.
