The Countywide Diabetes Service seeks a Band 6/7 Diabetes Specialist Nurse within RGN. You will support adults living with Type 1 and Type 2 diabetes. The nurse-led service focuses on assessment, management, care planning, education, and coordinated clinical support.

Key Responsibilities:

- Assess and manage adult patients with Type 2 diabetes
- Support non-complex Type 1 diabetes management
- Create and reassess individualized care plans
- Educate patients about diabetes self-management strategies
- Collaborate with multidisciplinary teams and medical consultants

Requirements:
- Current NMC registration with strong nursing credentials
- Significant diabetes care experience, preferably within specialist services
- Demonstrated adult Type 2 diabetes management experience
- Proficiency in clinical assessment and patient education
- Experience with non-complex Type 1 diabetes cases
