Key Responsibilities
• Provide professional social work and crisis intervention services to seriously mentally ill clients.
• Develop and implement creative treatment and recovery plans, discharge planning, support services and programs responsive to clients’ needs.
• Engage in client-centered care.
• Collaborate with the inter-professional team.
• Maintain appropriate documentation and data collection, in accordance with hospital, college and legislative standards.
• Participate in the development and delivery of educational and information sessions for clients, families, service providers, and the community.
• Participate in Social Work meetings, peer consultation and committees.
• Keep up to date regarding professional standards by reviewing literature, reading articles, attending educational events and networking with colleagues
• To work in a manner that is in compliance with staff and patient/client safety practices, policies and procedures of the ROHCG.
• To ensure a work environment that is conducive to the ROHCG Harassment Free policy. Qualifications
• Bachelor of Social Work from an accredited university.




• Three years’ experience in a mental health setting.
• Member in good standing with the Ontario College of Social Workers and Social Service Workers.
• Knowledge, skills and training in concurrent disorders.
• High level of knowledge, skill and confidence in using electronic health record systems for clinical documentation and navigation.
• Knowledge of community resources, corrections, and applicable legislated acts (MHA, PHIPA, The Health Care Consent Act, etc.).
• Understanding of recovery-based treatment and programs of clients with severe and persistent mental illness.
• Demonstrate ability to work both independently and in a professional inter-disciplinary team workplace.
• Strong advocacy & conflict resolutions skills.
• Strong organizational skills and work ethic, with excellent oral and written communication skills. Language Requirements English level A
