Fraser Health Authority is seeking a skilled Mental Health & Substance Use Services liaison for the Primary Care Networks in British Columbia. You will assess, screen, and refer clients, provide brief counselling, and support transitions between PCNs and MH&SUS; SCSP teams.

You will also advocate for Indigenous clients, coordinate with community resources, and deliver staff training while maintaining records and adhering to clinical standards.
