The PHC RN functions within the population health promotion model while providing preventative, maintenance of wellness; and illness and injury prevention services. The PHC RN collaborates and makes appropriate referrals to other health care professionals, initiates and/or participates in community development initiatives and participates in the evaluation of PHC initiatives within the site.
