Key Responsibilities:

- Assess clients for admission and establish helping relationships
- Perform multidimensional assessments and identify crisis intervention needs
- Develop care plans with clients, families, and professionals
- Authorize services and connect clients with community resources
- Monitor plans, reassess needs, and complete discharge planning

Requirements:

- Registered nursing or regulated allied health professional qualification
- Registration with the applicable Ontario professional college
- Related qualified experience, normally attained over two years
- Valid driver’s license and access to a vehicle
- Vulnerable Sector Check completed within six months
