- $64.19 / hour. We are seeking a Clinical Pharmacist to join our team at Abbotsford Regional Hospital and Mission Memorial Hospital located in Abbotsford and Mission, BC. We are a collaboration of four premier health employers: Fraser Health, Provincial Health Services, Providence, and Vancouver Coastal health. We provide an integrated approach to pharmaceutical care and service excellence for our clients. As a Clinical Pharmacist, you will work to provide the following services to our patients: Participating in medication use evaluations, quality improvement initiatives, and clinical drug trials Providing mentorship and supervision to pharmacy students, residents, and new staff Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience

The Clinical Pharmacist provides clinical pharmacy services, including individualization of patient medication therapy, patient medication counselling, and consultation with physicians, nurses, and other health care providers to develop and discuss therapeutic plans. Participating in ward rounds, program team meetings, and provision of pharmacy services at the unit level Contribute to quality improvement activities Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience Broad knowledge of organization policies, procedures, and standards of care Knowledge of evidence-based clinical pharmacy practice Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision Ability to communicate effectively both verbally and in writing About Fraser Health: We are committed to planetary health and value diversity in the workforce.
