About Concierge Care Calgary

Concierge Care Calgary is a locally owned, nurse-led home care company dedicated to helping individuals remain safe, comfortable and independent in their own homes.

Founded by four local women, including three Registered Nurses, our leadership team brings over 100 years of combined nursing experience to the care we provide.

We are not a franchise or a traditional home care agency. We believe exceptional care starts with strong relationships, thoughtful caregiver matching, consistent caregivers and professional nursing oversight.

As our company continues to grow, we are looking for an experienced, compassionate and highly organized Registered Nurse to join our team as a Care Manager.

About the Opportunity

This is a unique chance for an RN who enjoys combining clinical nursing knowledge with client relationships, caregiver support and care coordination.

Unlike a traditional nursing position, this role involves a combination of client assessments, care planning, caregiver supervision, family communication and hands-on nursing support when needed.

The position will begin on a part-time basis, with a guaranteed minimum of 20 hours per week and an anticipated 20–25 hours initially. Our intention is to grow the role into a full-time position as the successful candidate becomes familiar with our clients, caregivers and operations, and as our business needs develop.

Compensation will be reviewed as the Care Manager gains experience with CCC, assumes additional responsibilities and demonstrates the ability to independently manage a growing client portfolio.

Key Responsibilities

Client Care & Clinical Oversight

- Conduct comprehensive nursing assessments for new and existing clients.
- Develop, implement and regularly review individualized, client-centered care plans.
- Monitor changes in client health, safety and care needs, making recommendations and adjustments as appropriate.
- Provide nursing visits and clinical interventions within scope of practice when required.
- Support clients with complex care needs, including dementia, chronic illness, mobility limitations and palliative care.
- Communicate with families, healthcare professionals and other members of the care team.

Caregiver Support & Supervision

- Introduce caregivers to new clients and help ensure a comfortable, successful transition.
- Provide clinical guidance, education and support to Health Care Aides and Licensed Practical Nurses.
- Help ensure caregivers understand each client's needs, preferences and care plan.

- Participate in caregiver competency assessments and ongoing education.
- Address care concerns and provide constructive feedback when needed.
- Work collaboratively with the scheduling team to support continuity and appropriate caregiver matching.

Care Coordination & Quality Assurance

- Maintain accurate, timely clinical documentation and care plans.
- Conduct regular client follow-ups and quality assurance visits.
- Respond professionally to client and family questions or concerns.
- Collaborate with CCC's owners, office team and other Care Managers to ensure a consistently high standard of care.
- Identify opportunities to improve care processes, caregiver education and the overall client experience.

Hours & Scheduling We recognize that home care doesn't always fit neatly into a traditional 9-to-5 schedule.

Most hours will be during regular daytime business hours. However, our clients' needs occasionally require flexibility, such as introducing a caregiver during an evening or weekend, responding to a change in a client's condition, or assisting with an urgent care situation.

Rarely, an urgent client need may require an after-hours nursing visit, particularly when supporting clients receiving palliative care.

We are looking for someone who understands the nature of community-based nursing and is comfortable with some scheduling flexibility.

We also value work-life balance and will work to plan visits in advance whenever possible, share responsibilities appropriately and establish clear expectations for after-hours coverage.

The successful candidate will not be expected to be continuously available 24/7.

Qualifications

Required:

- Active registration and practice permit in good standing with the College of Registered Nurses of Alberta (CRNA).
- Minimum 3 years of relevant nursing experience.
- Strong clinical assessment, care planning and critical-thinking skills.
- Excellent communication, organization and documentation skills.
- Ability to work independently and make sound clinical decisions.
- Valid driver's licence, reliable vehicle and willingness to travel to clients' homes throughout Calgary and surrounding areas.
- Ability to accommodate occasional evening or weekend visits and participate in an agreed-upon after-hours coverage arrangement.

Preferred:

- Experience in home care, community nursing, case management or care coordination.
- Experience with dementia, palliative care and complex client needs.
- Experience supervising, mentoring or educating caregivers.
- Familiarity with Alberta's home care system and Client Directed Home Care Invoicing (CDHCI).

- Experience working with electronic care records and scheduling systems.

What We're Looking For Our ideal candidate is a nurse who genuinely enjoys people and takes pride in providing exceptional care.

You are compassionate, confident and approachable. You understand that home care involves more than clinical tasks; it means getting to know clients and their families, recognizing what matters to them and helping caregivers deliver care that respects their individuality.

You are comfortable making decisions, solving problems, communicating with families and supporting a team of caregivers.

Most importantly, you care deeply about the quality of care clients receive.
