Ontario Health atHome Champlain seeks a Care Coordinator for its Hospital Care Team. You will assess eligibility, plan services, authorize care, and coordinate referrals. The work supports safe transfers home while engaging patients, caregivers, families, and hospital partners.

Key Responsibilities:

- Assess clients for admission and establish helping relationships
- Perform multidimensional assessments and identify crisis intervention needs
- Develop care plans with clients, families, and professionals
- Authorize services and connect clients with community resources
- Monitor plans, reassess needs,



and complete discharge planning

Requirements:

- Registered nursing or regulated allied health professional qualification
- Registration with the applicable Ontario qualified college
- Related professional experience, normally attained over two years
- Valid driver’s license and access to a vehicle
- Vulnerable Sector Check completed within six months
