Support youth with complex health needs as a Community Health Nurse at Providence Health Care, using assessment and individualized care planning. Work includes primary care, mental health, and substance use support.
This Community Health Nurse provides comprehensive care for youth and young adults. You will assess clients, develop nursing care plans, deliver education, and support transitions. The position also connects clients with community resources while collaborating with interdisciplinary teams under BCCNM standards, Providence policies, and clinical protocols.
Key Responsibilities: Assess clients through interviews, observations, charts, and interdisciplinary input
Develop, implement, and evaluate individualized nursing care plans
Provide community outreach and primary care nursing services
Perform wound care, testing,



phlebotomy, and pelvic exams
Document progress, interventions, and transition plans accurately
Requirements: Current practicing registration with BCCNM as a Registered Nurse
Two years’ recent experience supporting youth and young adults
Primary care and mental health or substance use experience
Point of Care HIV Testing course completion
BCCDC Immunization certificate and reproductive health certification or enrollment
Bring youth nursing, health education, harm reduction, and interdisciplinary collaboration skills to Providence Health Care’s community services.
