The Bundled Care Coordinator supports patients undergoing primary unilateral hip or knee replacement across the care continuum. This healthcare professional communicates with families and inter-professional teams, advocates for patient needs, provides evidence-supported education, and connects patients with resources. The position also supports outcome data collection, reporting, and program initiatives.
Key Responsibilities: Coordinate transitions across the total joint replacement care continuum
Act as liaison among patients, families, hospitals, and community resources
Identify barriers, discuss preferences, and advocate for patient choices




Connect patients with healthcare teams and appropriate resources
Document records and submit patient and program outcome data
Requirements: Degree in a health-related field
Regulated health qualified in good standing with respective College
Knowledge and experience with musculoskeletal diagnoses and total joint replacement
Project management or event coordination work experience
Experience using Excel, electronic documentation, and data reporting
Support informed patient participation through total joint replacement coordination, education, advocacy, communication, and outcome reporting.
