EHN Canada’s Guardians Care Specialist owns the referent experience across inpatient and outpatient mental health services. You will coordinate referrals, admissions, treatment progression, and discharge planning. The work connects clients, referents, clinical teams, site leadership, and Business Development while maintaining service standards, reporting obligations, and contractual commitments.
Key Responsibilities: Coordinate inquiries and referrals from assigned partner organizations
Manage screening, admission planning, and pre-admission activities
Serve as primary referent contact throughout treatment
Monitor milestones,



reporting deadlines, and discharge documentation
Maintain accurate records and conduct file audits
Requirements: Bachelor’s degree in a related field preferred
Experience in healthcare, mental health, or case coordination
Experience within interdisciplinary healthcare environments
Proficiency with CRM systems and electronic health records
Solid communication, organization, and stakeholder engagement skills
Apply relationship management, care coordination, and problem-solving skills to strengthen referent experiences throughout EHN Guardians treatment journeys.
