Support vulnerable individuals and families as a Housing Social Worker with the City of Windsor, providing intensive case management and counselling. This regular or temporary full time position requires shift work.
As a Social Worker, Housing, you will serve clients experiencing homelessness or housing vulnerability. Your work includes case planning, crisis intervention, bio-psychosocial assessment, community referrals, and risk assessment. You will collaborate with health, justice, housing, and social service partners.
Key Responsibilities: Provide intensive on-site and in-home case management
Create and regularly update client case plans
Deliver crisis intervention and clinical counselling
Assess needs and make community resource referrals
Coordinate interdisciplinary case conferences for complex needs
Requirements: Four-year university degree in Social Work
Over one year of relevant work experience
Registration with Ontario's social work college
Ability to travel to offsite locations
Microsoft Suite experience in computerized office environments
