Key Responsibilities: Assess patient and family care needs
Determine eligibility and develop individual service plans
Link patients with community service providers
Coordinate and monitor care plan delivery
Respect patient values, choices, and preferences
Requirements: Current registration with an applicable Ontario regulatory body
Two or more years of recent community health experience
Knowledge of health care systems and community resources
Solid assessment, communication, and decision-making skills
Valid driver’s licence and reliable vehicle access
Bring regulated clinical expertise, care coordination, and community health knowledge to patient-centred services in East Mississauga.
