Fraser Health is hiring a Licensed Practical Nurse for Community South Fraser. This position manages assigned client caseloads involving complex chronic conditions, geriatric patients, care planning, health counselling, and transitions. The work supports clients and families living safely at home or in the community.
Key Responsibilities: Complete individualized assessments and interprofessional care planning
Provide direct nursing care and therapeutic interventions
Coordinate referrals, services, transitions, and follow-up




Educate clients about self-management and health promotion
Maintain clinical records and statistical information
Requirements: Current practicing BCCNM registration as a Licensed Practical Nurse
One year of recent related clinical experience
Experience assessing complex geriatric or adult chronic conditions
Valid BC driver's license and personal vehicle access
Ability to work independently and interprofessionally
