Support crisis recovery as a Transitional Case Worker in Clinton, Ontario, using assessment and crisis intervention skills. This is a temporary part-time position.
Huron Perth Healthcare Alliance seeks a Tech 5 Transitional Case Worker for its Clinton Safe Bed home. You will support individuals experiencing serious mental illness or addictions-related crises. The work includes intake, assessment, short-term crisis counselling, referrals, advocacy, and coordinated community support. You will also help maintain a safe therapeutic environment and participate in practical home operations.
Key Responsibilities: Assist intake, assessment, and support for Secure Bed clients
Provide crisis intervention and short-term crisis counselling
Connect clients with community resources and referrals




Complete treatment and crisis plans with partners
Support daily operations of the Safe Bed home
Requirements: Registered Social Service Worker or Addiction Counsellor
Current registration and licensure with an Ontario college
Minimum three years progressive clinical experience
Knowledge of mental health and addiction services
Valid Ontario driver’s license, transportation, and required insurance
Apply clinical assessment, crisis intervention, and advocacy skills while supporting clients through Safe Bed recovery in Clinton.
