SE Health seeks a Physiotherapist for its Geriatric Outreach Program and Senior Wellness Clinic. You will manage patients through assessment, treatment, consultation, and follow-up services. The work includes client-centered treatment planning, goal formulation, and collaboration with interdisciplinary team members.
Key Responsibilities: Conduct patient assessments across outreach and clinic settings
Formulate patient goals and clinical recommendations
Plan client-centered, goal-directed treatment programs
Implement treatment programs for patients
Provide consultation and follow-up services
Requirements: Recent acute or rehabilitation patient management experience
Geriatric population experience preferred
Effective interdisciplinary collaboration and communication skills
BCLS and CPR certificate
Valid driver’s license, vehicle, and travel ability
Apply physiotherapy assessment, treatment planning, and geriatric care skills across patients’ homes and Richmond Hill Hospital.
