The Countywide Diabetes Service seeks a Band 6/7 Diabetes Specialist Nurse within RGN. You will support adults living with Type 1 and Type 2 diabetes. The nurse-led service focuses on assessment, management, care planning, education, and coordinated clinical support.
Key Responsibilities: Assess and manage adult patients with Type 2 diabetes
Support non-complex Type 1 diabetes management
Create and reassess individualized care plans
Educate patients about diabetes self-management strategies
Collaborate with multidisciplinary teams and medical consultants
Requirements: Current NMC registration with robust nursing credentials
Significant diabetes care experience, preferably within specialist services
Demonstrated adult Type 2 diabetes management experience
Proficiency in clinical assessment and patient education
Experience with non-complex Type 1 diabetes cases
