Fraser Health seeks a Social Worker with acute care clinical experience and comprehensive counseling skills. You will assess psychosocial needs, provide therapeutic services, and guide patient care planning. The work includes consultation, referrals, documentation, and interdisciplinary collaboration.
Key Responsibilities: Conduct comprehensive social work and psychosocial assessments
Provide therapeutic clinical counseling to patients and families
Lead discharge planning and interdisciplinary care coordination
Connect patients with community resources and referrals




Document assessments, progress notes, reports, and patient information
Requirements: Master’s degree in Social Work from an approved school
One year recent related acute care clinical experience
Current full registration with the BC College of Social Workers
Comprehensive counseling skills, including crisis and grief support
Ability to work independently within multidisciplinary teams
Use advanced assessment, counseling, and care-planning skills to support patients and families at Surrey Memorial Hospital.
