Coordinate mental health referrals remotely for EHN Canada, using relationship management and stakeholder communication to guide clients from inquiry through discharge.
As a Guardians Care Specialist, you own the referent experience for an assigned caseload across inpatient and outpatient services. You coordinate admissions, treatment progression, reporting, and discharge planning while working with clients, referents, clinical teams, and Business Development. This temporary full-time 12-month position is currently remote, with in office attendance subject to operational needs.
Key Responsibilities: Coordinate inquiries and referrals from assigned partner organizations
Manage screening, admission planning, and pre-admission activities
Advance referrals within established service standards




Coordinate treatment progression and discharge planning
Maintain referent communication, reporting, and contractual commitments
Requirements: Experience in healthcare, mental health, or relationship management
Experience within interdisciplinary healthcare environments
Proficiency with CRM systems and electronic health records
Excellent verbal and written communication skills
Ability to manage priorities and sensitive situations
Bring stakeholder engagement, care coordination, and sound judgment to EHN Guardians while supporting timely access to mental health services.
