As a Licensed Practical Nurse on the Integrated Care Team at Downtown Community Health Center, you will provide client and family-centred care. Work across clinics, sites, services, and outreach or home-based visits. Support people experiencing complex health challenges through primary care, mental health, substance use, and community resources.
Key Responsibilities: Assess, plan, implement, evaluate, and document client care
Provide holistic care across the primary care continuum
Build therapeutic relationships with clients and families
Collaborate with interdisciplinary providers and Community Care Services




Seek direction when client needs are unstable or unpredictable
Requirements: Graduate from a recognized Practical Nursing program
Hold current BCCNM registration as a Licensed Practical Nurse
One year recent related experience or community clinical placement
Knowledge of Trauma Informed Practice, Harm Reduction, and Recovery Oriented Care
Demonstrated interdisciplinary communication, judgment, and computer skills
Bring culturally secure nursing, clinical judgment, and collaborative care skills to Vancouver Coastal Health’s Downtown community team.
