Provide person-centered nursing care as a part time Licensed Practical Nurse with Extendicare, using clinical judgment and care coordination skills.
Reporting to the Director of Care or Director of Health & Wellness, the LPN delivers planned, patient-oriented, goal-directed care. You will assess residents, maintain documentation, administer treatments, and coordinate services according to provincial standards. The position supports long-term care residents through cooperative practice.
Key Responsibilities: Complete resident assessments, care plans, and clinical documentation
Provide direct care and administer medications and treatments
Evaluate care plans and communicate required changes
Coordinate assignments, conferences, transfers, and resident services




Manage significant condition changes and medical emergencies
Requirements: Current Licensed Practical Nurse registration in applicable province or state
Strong communication, critical thinking, and care coordination skills
Sound clinical knowledge and customer focus
Current Basic Cardiac Life Support and First Aid certification
Computer proficiency with Microsoft Office applications
