Support perinatal patients through assessment, counseling, and community referrals as a Social Worker with Sinai Health in Toronto. This temporary full time position serves the Mount Sinai Academic Family Health Team.

The Social Worker provides psychosocial, emotional, and mental health support within an academic family medicine setting. Responsibilities include crisis intervention, psychotherapy, case management, psycho-education, and consultation. The role also contributes to program delivery, clinical research, learner education, and collaborative patient care.

Key Responsibilities:

- Assess psychosocial problems and provide social work diagnoses
- Deliver individual, couple, family, and group counseling
- Manage cases and connect patients with community resources




- Implement mental health screening and assessment tools
- Participate in case rounds and program development

Requirements:

- Master of Social Work from an accredited institution
- Current good-standing registration with the Ontario College
- Three years of direct client service experience
