Lead clinical practice across hospital partnerships with VHA Home HealthCare in Ontario, supporting client safety, outcomes, and care delivery.

As Supervisor, Hospital Partnerships, you provide program and clinical support, in-home supervision, and practice oversight. You monitor standards, resource utilization, and key performance indicators for regulated and unregulated services. Working with interdisciplinary teams, you provide consultation and clinical leadership that strengthens client and family-centred care.

Key Responsibilities:

- Provide clinical leadership, mentorship, consultation, and supervision
- Oversee practice and performance for point-of-care staff
- Monitor client care needs, outcomes, and practice standards




- Promote client safety and risk reduction with leadership teams
- Support organizational change and departmental deliverables

Requirements:

- RN diploma or degree from a recognized institution
- Registration in valuable standing with the College of Nurses of Ontario
- Three to five years of management experience
- Community or home care services knowledge
- Valid driver's license, vehicle, and on-call availability
