Deliver clinical pharmacy services as a Provincial Relief Pharmacist with Sobeys Inc., supporting patients, practitioners, and retail pharmacies across an assigned territory.
This pharmacist provides patient-centered care while working shifts scheduled by the Relief Coordinator. The role emphasizes disease state management, medication reviews, health risk assessments, prescription safety, and customer service. You will support workflow efficiencies and skilled clinical services across Sobeys, Safeway, Thrifty Foods, Foodland, Lawtons Drugs, or FreshCo pharmacies.
Key Responsibilities:
- Provide pharmacy services to customers, practitioners, and retail pharmacies
- Initiate care plans, follow up, and document patient information
- Conduct medication reviews and health risk assessments
- Counsel patients on therapies and medication adherence
- Identify drug-related problems and prioritize prescription safety
