Fraser Health seeks a Clinical Pharmacist for an assignment available until approximately December 2026 or the incumbent’s return. The position provides individualized pharmacotherapy, patient counselling, therapeutic planning, and drug distribution services. It also supports medication use evaluations, quality improvement, clinical drug trials, and pharmacy education.
Key Responsibilities:
- Individualize medication therapy using charts, laboratory results, and clinical responses
- Counsel patients and families about medication therapy and procedures
- Develop therapeutic plans with physicians and health care providers
- Validate prescriptions,
resolve safety concerns, and dispense medications accurately
- Mentor pharmacy students, residents, and new staff
Requirements:
- Current registration with the College of Pharmacists of British Columbia
- Accredited Hospital Pharmacy Residency Program completion or equivalent
- Knowledge of evidence-based clinical pharmacy practice
- Knowledge of medication use evaluation and research methodology
- Ability to communicate effectively verbally and in writing
