- Brantford is looking for a compassionate and dedicated Registered Nurse to join their growing team in Brantford, ON. Responsibilities:
• Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications, treatments, and nursing interventions in a home care setting
• Monitor and document patient health status, progress, and changes in condition
• Collaborate with physicians, healthcare teams, and family members to coordinate patient care
• Provide patient and family education on health management, medications, and disease prevention
• Perform wound care, IV therapy, catheter care, and other clinical nursing procedures




• Maintain accurate and timely clinical documentation in compliance with regulatory standards

Requirements:
• Current Registered Nurse (RN) registration with the College of Nurses of Ontario (CNO)
• Previous home care, community health, or clinical nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills for patient and family interactions
• Ability to work independently and manage a caseload with minimal supervision
• Valid driver's license and reliable vehicle for home visits in the Brantford area
• Current CPR/First Aid certification
