Provide clinical counselling in Psychiatry with Fraser Health, supporting patients and families affected by acute mental illness and substance misuse.

This Social Worker provides therapeutic services within an integrated interdisciplinary team. The position includes psychosocial assessments, crisis counselling, care coordination, discharge planning, and consultation. You will also support patients, families, healthcare professionals, and community agencies through referrals and education.

Key Responsibilities:

- Deliver clinical counselling to patients, families, and treatment teams
- Complete psychosocial assessments using varied skilled modalities
- Coordinate care plans, referrals, and discharge planning
- Document assessments, progress notes, reports, and patient information




- Provide consultation, education, and support regarding mental health

Requirements:

- Master's degree in Social Work from an approved school
- Two years' recent clinical mental health experience
- Current full registration with the BC College of Social Workers
- Comprehensive counselling skills, including crisis interventions and group work
- Ability to communicate effectively with patients, families, and professionals
