As a Provincial Relief Pharmacist, you will provide patient-centered care across Sobeys, Safeway, Thrifty Foods, Foodland, Lawtons Drugs, and FreshCo pharmacies. Your work emphasizes disease state management, medication reviews, health risk assessments, prescription safety, and counselling. You will also support workflow efficiency and professional clinical services.
Key Responsibilities:
- Provide clinical services, medication reviews, and health risk assessments
- Initiate, document, and follow up on patient care plans
- Identify drug-related problems and monitor medication adherence
- Counsel patients about new therapies and medication purposes
- Deliver patient-focused service across assigned relief shifts
