Community Outreach LPN - OAT & Medication Support Pill4Me is a community pharmacy in Chilliwack focused on improving access to medications and healthcare services. We work closely with patients, prescribers and community organizations, including supporting individuals receiving opioid agonist treatment (OAT) and other substance-use care. We are looking for aLicensed Practical Nurse (LPN)to join our community outreach team.

This is a field-based role focused on medication delivery and supporting patients who receive OAT and other prescribed therapies in the community. The LPN will work closely with the pharmacist while independently completing scheduled community visits throughout Chilliwack. Deliver medications to patients in the community Support patients receiving OAT and other prescribed therapies Coordinate with pharmacists,



prescribers and community healthcare partners as required Current registration as a Licensed Practical Nurse (LPN) in good standing in British Columbia Comfortable working independently in community settings Professional and non-judgmental approach to substance-use care Experience with OAT, substance-use care, mental health, harm reduction or community nursing is an asset Familiarity with electronic documentation and mobile technology is an asset Versatile and predictable community-based hours Training and orientation specific to pharmacy delivery and OAT workflowsOpportunity to work independently while supported by a pharmacist and pharmacy team Opportunity to help develop an expanding community healthcare program Part-time ~ Flexible, based on patient delivery requirements #
