The Nurse Case

Manager CM-Nurse, handles intake callsin a qualified and effective manner,conducting home assessments, nursing assessments and wellnesschecks with clients and their families. The CM-Nurse has the primary responsibility of obtaining clients, conducting assessments of client’s needs, interacting with families to ensure quality care and expectations are managed and delivered. The goal of AHH services is self-actualization of the patient and encourage the caregivers to do the same to promote optimal health and well-being.
