Join Fraser Health as a Licensed Practical Nurse Outreach, providing critical care for clients with mental health disorders. This role involves working both in the community and at home, focusing on patient-centered support. As a member of a multidisciplinary team, the LPN Outreach conducts assessments, administers medications, and plans patient care collaboratively with RNs and RPNs.

- Administer medications following policies and procedures
- Assess and monitor patient strengths and limitations




- Contribute to client care plans and interdisciplinary meetings
- Organize patient care and respond to changing needs
- Document care information to ensure patient safety Requirements:
- Graduate from a recognized Practical Nursing program
- Current LPN registration with BCCNM
- First Aid, CPR, and Food Protected certifications
- Valid BC Driver's License and access to a vehicle
- Ability to communicate and work independently Deliver personalized nursing care and participate in quality improvement initiatives as part of Fraser Health's dedicated team.
