Conducting and completing the detailed Community Mental Health Assessment and SBAR notes for individuals referred

- Assessing, screening, referring and linking individuals to internal CMHA services or alternate community resources
- Exploring and linking individuals to internal CMHA services or alternate community resources, when needed
- Consulting with the Mental Health First Response/RAIT Team to prioritize individuals awaiting service




- Providing intervention and brief solution focused therapy support to individuals experiencing a psychiatric crisis (intervention may be via telephone or by personal contact)
- Linking individuals with urgent needs to immediate resources (ie securing food resources, financial assistance, and housing). Post-secondary degree in Social Work from an accredited university with current registration with the O.Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.or Bachelor’s Degree with an applicable college diploma in Addictions and current registration with the C.Preference will be given to a qualified candidate who is proficient in both official languages.
