Ontario Health atHome is seeking a Temporary Float Hospital Care Coordinator to assess patient care needs, determine eligibility, and connect patients with community services across hospital sites in Ontario Health atHome Toronto Central.You will lead patient-centered care, coordinate with families and health care teams, and support care plans while rotating shifts of eleven hours from 09:00 to 20:00, biweekly three and four days, including every second weekend.
