Make a meaningful impact as a Community Care Navigator at Dorothy Ley Hospice, working full-time with an equity-informed, person-centered approach. Support individuals in navigating complex health systems while ensuring timely intake and administrative coordination.As a key member of the Community Care team, you'll manage general administration and coordinate intake processes, assisting both healthcare professionals and the community. You will conduct initial screening for palliative, caregiver, and bereavement needs, while also reducing barriers to care.



Collaborate closely with program staff and volunteers to enhance service delivery and support community members effectively.Key Responsibilities:Process current referrals accurately and timely in EMRServe as the primary contact for inquiries from health professionalsConduct registration and intake screenings for community referralsProvide navigational support for health and social servicesSupport volunteer program administration and coordinationRequirements:BSW or equivalent experience in social workMinimum two years with individuals facing life-limiting illnessesProficient with electronic health recordsStrong interpersonal and communication skillsExperience in hospice or health care settings preferredBring your expertise and compassion to support community members at Dorothy Ley Hospice.
