Title: Homeshare Liaison
Reports To: Homeshare Coordinator, Programs Director
Employment Type: Casual (Non-Management)

Roles and Responsibilities

- Oversee and support a caseload of 5-6 persons
- Conduct Quarterly and Annual Monitoring visits
- Complete dedicated forms
- Check in with the supported person and homeshare providers
- Discuss current SMART goals
- Room/suite check
- Summarize any visits or interactions with person supported, homeshare providers, CLBC, families and add notes to Contact Logs in each person's PIB (on Server)
- Partake in any planning/progress meetings regarding the person
- Personalized Support Planning (PSP) and Smart goals
- Conduct PSP meeting with person supported, family members they want involved, HSPs, staff and CCL Team
- Go over quarterly SMART goals and record monthly outcomes tracker (all on server)
- Preference meetings
- Organize and meet potential homeshare intakes
- Introduce them to 2-3 new Homeshare providers to see which may be the right fit
- Discuss outcomes/updates with Homeshare Coordinator
- You would be responsible for updating the Personal Information Binder (PIB) of each person on your caseload




- Update PIB annually (personal info, PSP, med guidelines updates, etc.) (all on server)
- Ensure all HSP documents are up to date

Qualifications:

- Certificate/Diploma in Community Support, Health Care, Social Work, Psychology, or related field.
- At least 2 years of community support experience.
- Robust communication, leadership, and organizational skills.
- Valid Class 5 Driver’s License, First Aid/CPR, and clear Criminal Record Check.
- Experience in the Community Living sector preferred.

Non-management leadership role reporting to the Programs Director and Homeshare Coordinator.
