CUPS Calgary is seeking a C2C Health Navigator to deliver trauma-informed case management for participants with complex health and social needs. You will develop person-centered care plans and advocate for dignity within health and social services.

Based onsite at Calgary, you will join the C2C/CAMPP team, working full time with a focus on community integration and resource navigation for vulnerable populations.
