Ontario Health atHome is seeking a Care Coordinator in Lindsay to assess patient needs, arrange services, and connect patients with community resources. You will lead care planning and coordinate with caregivers and providers in a hospital-assigned Point of Access program.

The role supports patient-centered, accessible care in home or community settings. Requires active Ontario licenses (RN/Physiotherapist/OT/RSW) with 2+ years in community health.
